Job Description:

The Executive Director (ED), Continuing Care is a senior leadership role responsible for shaping the future of continuing care across Integrated Northern Health. Guided by the Saskatchewan Health Authority mission, “We care. We work together to improve health and well-being; every day, for everyone,” ED provides strategic and operational leadership to strengthen continuing care services, improve quality, safety and access, and support the vision of Healthy People, Healthy Saskatchewan.

Reporting to the Vice President, Integrated Northern Health, the Executive Director contributes to provincial and northern strategy by identifying system gaps, emerging population health needs and opportunities to strengthen continuing care services across northern Saskatchewan. The role advances SHA’s CARES values of Compassion, Accountability, Respect, Equity and Safety by creating culturally safe, inclusive and high-performing environments for residents, families, communities, physicians, providers and staff.

The Executive Director provides senior leadership across remote and northern communities where innovative service delivery, strong partnerships and creative workforce solutions are essential to improving access, quality and health outcomes. The role translates SHA strategic priorities and roadmap goals into actionable operational plans, ensuring effective implementation, performance monitoring, continuous improvement and measurable results. This includes strengthening continuing care services, supporting recruitment and retention, modernizing care delivery and scope of practice, improving safety and quality, strengthening service integration, and ensuring responsible stewardship of people, financial and physical resources.

The Executive Director works collaboratively with senior leaders, physician partners, Indigenous communities and organizations, affiliate organizations, community partners, government stakeholders and interdisciplinary teams to support coordinated, high-quality continuing care services across northern Saskatchewan. Together, these partnerships improve health outcomes, reduce barriers to care and advance Saskatchewan Health Authority priorities.

Human Resources Exemption: No

Experience

  • Ten (10) years of progressive leadership experience in a complex health system, including experience leading interdisciplinary teams, system transformation, service integration and operational performance.

  • Demonstrated experience in strategic and operational planning, financial stewardship, quality improvement, change management and partnership development.

  • Experience working with remote, northern and Indigenous communities is preferred.

Other Education and Training:

  • Undergraduate degree or an equivalent combination of education and experience
  • Master's degree preferred

  • Lead the planning, delivery and evaluation of continuing care services across northern communities to improve quality, access, continuity and resident/client experience.

  • Advance team-based models of care that strengthen transitions across the continuum of care and support residents and clients in receiving the right care, in the right place, at the right time.

  • Establish clear performance expectations, monitor outcomes and use data to drive quality improvement, accountability and service redesign.

  • Champion culturally safe care, Truth and Reconciliation, health equity and meaningful engagement with Indigenous communities and partners.

  • Build and sustain effective relationships with patients, families, staff, physicians, affiliate organizations, unions, community partners, government and other stakeholders.

  • Ensure effective resource management, risk mitigation, workforce planning and compliance with legislation, policy and organizational standards.
