Job Description: Under the direction of the Program Manager responsible for Intake, and as part of an interdisciplinary team, the Assessor Coordinator is responsible to respond to initial requests for information and services across Mental Health and Addiction sites. The Assessor Coordinator will provide a range of screening, triage, assessment, treatment and case management services to a population of acute and chronic mentally disordered adults as well as children /youth and their families. The incumbent will provide program consultation and coordination of service including prioritizing and assigning to multidisciplinary staff or referring to community agencies . Also provides professional and public education regarding mental health issues and resources. "Preference for this position will be given to HSAS members in accordance with the terms and conditions of the SAHO/HSAS Collective Agreement"

Human Resources Exemption: No

Experience

  • Experience working with adults/kids/families in community mental health setting
  • Min 1 year postgrad recognized clinical exp in community mental health setting
  • 1 year experience in the area of mental health and addiction services

Education

  • Recognized degree in nursing , or a social work degree.

Licenses

  • Valid Class 5 driver's license

  • Nursing diploma is acceptable
