Job Description: This position will be part of the Intermediate Care Service within the Primary Health Care Networks. This service addresses a provincial health system service gap, where some seniors (adults aged 65+) with multiple complex health issues have difficulty in accessing an office based primary care provider. The goals for this service are: to provide the most at risk seniors with home based access to higher level health care providers, including MDs and NPs; to mitigate the risk for worsening health states and acute care episodes leading to ED visits and hospitalizations for at risk seniors; to reduce system pressures on acute care services, particularly EDs and hospitals; to provide cost effective and high quality health care using a patient centered and community based approach. Practitioners in this service are assigned to one of the Primary Health Care Networks and provide support to the interdisciplinary Primary Health Care team in client care provision not exclusive to the seniors' population.
Human Resources Exemption: No
Experience
- 3 years experience as an RN in primary care, public health or acute care
- One year experience in community needs assessment/development/health promotion
Education
- Graduate from an approved School of Nursing
Licenses
- Valid Class 5 driver's license
- Registered Nurse (RN)
Other Education and Training
- Registered/eligible for registration with CRNS as a Nurse Practitioner (NP)
- Ability to maintain current registration as a Nurse Practitioner with CRNS
- Basic Cardiac Life Support Certificate (BCLS)

- Knowledge in population and public health concepts
- Ability to apply methods and principles of nursing process
- Demonstrates capacity to plan, implement and evaluate multiple assessments

- Demonstrates capacity to work closely with multiple physicians
- Demonstrate responsible decision-making supported by strong critical thinking
- Demonstrates knowledge of community services, programs, and resources
- Knowledge of common medical conditions and treatments.
- Knowledge in individual assessment and counselling skills
- Demonstrates ability to assess the community for primary care needs.
- Demonstrates ability to show initiative and self-direction
- Knowledge in the principles of Population Health Promotion
- Demonstrates ability to accept and implement change
- Demonstrates leadership, confidence and autonomy in the RN (NP) role
- Demonstrates ability in verbal and non-verbal communication techniques
